PRESIDENT'S DIVERSITY INITIATIVE AWARD

Portland State University's Diversity Action Council is requesting nominations for the first annual President's Diversity Initiative Awards. The awards will be presented at the Council's annual Focus on Diversity Awards Reception to be held near the end of spring term (date and location TBA).
The Diversity Action Council wishes to recognize two faculty members, two students, and two staff members of the University who exemplify and honor the President's Diversity Initiative, which focuses on the following:

In addition to completing the fields above, please submit a one-page, typed response to the appropriate set of questions from the list below. Please also submit at least two letters of support from others who know the nominee well. These letters should address how the nominee exemplifies and honors the spirit of the Diversity Initiative. Finally, please submit any other relevant supporting documentation illustrating the nominee's contributions to diversity at Portland State University.
Faculty Questions:
1. How does the faculty member advocate on behalf of the Diversity Initiative
regarding issues that support ways to increase diversity of students, faculty,
and staff at the University?
2. How does the faculty member help create a welcoming, safe, and positive environment
for faculty, staff, and students of color at the University?
Staff Questions:
1. How does the staff member, through his or her daily activities, advocate
on behalf of the Diversity Initiative?
2. How does the staff member work with others to increase the number of persons
of color in his or her department or office?
Student Questions:
1. Through his or her involvements and engagements on campus, how does the student
advocate for the goals outlined in the Diversity Initiative?
2. How is the student involved in achieving the goals of the Diversity Initiative
on the campus, in the classroom, and in the community?
For each person nominated,
please send this completed form, a one-page response to the relevant set of
questions, at least two letters of support, and any other relevant supporting
documentation to the Center for Academic Excellence, 303 Cramer Hall, Portland
State University, to the attention of Kelle Lawrence, or you may submit materials
via e-mail to lawrencek@pdx.edu. To be considered, nominations must be received
